Financial Assistance

We strive to support those with healthcare needs who are uninsured, under-insured, ineligible for government programs, or unable to pay for medically necessary care.

How to Request Financial Assistance

  1. Complete the Lakes Regional Healthcare Financial Assistance Application.
  2. Submit the application along with recent copies of W-2s, tax returns, pay stubs, and bank statements to the clinic or hospital.
  3. Await a decision via mail.

Zero-Interest Financing Options

In-House Payment Plan

  • Conveniently pay off medical bills over up to 12 months with a $50 minimum monthly payment and 0% interest. Sign up easily by calling 712-336-8742.

Extended Payment Plan

  • Collaborating with Commerce Bank, we offer Health Services Financing (HSF®) with 0% APR, available regardless of insurance coverage and without credit checks. Terms of 24, 36, 48, or 60 months are available. Enroll quickly online or by phone.
